If you or a loved one is residing within a nursing facility or long term care facility and would like to return to the community Access Independence and the Money Follows the Person (MFP) Program can help!
Navigating disability benefits can be difficult, but our staff at Access Independence can provide assistance in determining eligibility, assisting with the application process and explaining employment options.
Safe Home Program
Safe Home is an innovative new program at Access Independence that enables persons aged 60 or over who are low-income and have disabilities, to maximize their independence, safety and security where they live.
Our Safe Home advocates are trained in benefits counseling, assistive technology and community services. They are available to help people get the most out of their benefits, learn about community services that enhance independence and evaluate their home for accessibility and safety.
We are also available to our community partners, housing and social service providers who want to maximize the independence and safety of older adults and persons with disabilities they serve.
